adformsctl.sh start and txkChkFormsDeployment.pl error

R12.1.3 environment running on RH Linux 5.10.

I just cloned this instance so I could start over with my app tier patching for Java updates and I don't believe I had an issue after cloning.

I then shutdown and applied a number of patches via help on MOSC from '@Bashar.

Now I am getting this error when starting.

Error while executing the perl script txkChkFormsDeployment.pl

We have determined that you need to redeploy Forms (using txkChkFormsDeployment.pl).

But could not do it automatically for you, due to some issues.
