HRMS RUP 8 20000288 worker fails on peaddasd.sql HR_PERSON_ADDRESS_BK3

We are at EBS R12.1.3 and already have HRMS RUP7 applied last year.  Trying to apply HRMS RUP 8 20000288.  The AD RUP 6 and FWK RUP 4 prerequisites appeared to apply successfully.  However, when I applied patch 20000288, I am getting worker failure on peaddasd,sql.  The adwork002.log shows the following:
