Siebel 8.1.1.14 Open UI custom Physical Render Files not working

AFter upgrading from Siebel 8.1.1.5 to Innovation Pack 2014 (8.1.1.14 Patchset 5), none of the custom Physical Renderer JS files seems to be working with Open UI. We tried implementing simple examples of Open UI customizations from blogs as well as ORacle training, but  in all cases the applet doesn't load after activating the custom manifest entries.  In one case, we tried implementing a custom PM and PR file to validate email address on Contact Form applet (Pretty much the example given at Siebel & Open UI: Open UI Customization - Part 4 - Applet PM and PR explained) . After activating the manifest entries, the files get downloaded to the browser, but applet doesn't load and we get errors like
