How to use OEM12cR4 to upgrade target DB 11.2.0.3 with ASM to higher version DB11.2.0.4?

Is anyone ever successfully using OEM12cR4 to upgrade target DB 11.2.0.3 with ASM to higher version, either 11.2.0.4 or DB12.1.0.x?

Per http://docs.oracle.com/cd/E24628_01/em.121/e27046/prov_db_overview.htm#EMLCM11112

We don't have a server with DB11.2.0.4 image on it, so we need to create a "Installation Media" for it. The Grid Infrastructure "Component" was created under "Database Provisioning", but it won't show up in the OEM upgrade screen.
