Outdated Patched Systems

I'm in a workshop that have some prod servers which have not been patched for  years. 

Are there any recommended documents for a gradual patch update for those type servers that are really for outdated?

On some of the servers I can do LU upgrade but there are some which only have 1 system disk.  Any assistance would be very much appreciated.
