RAC 11.2.0.3.0 upgrade to 11.2.0.3.14 failed

Hi, I am trying to 11.2.0.3.0 upgrade to 11.2.0.3.14 failed on RHL 6.5. I am using auto option and it got failed after patching GI home. Please help me to resolve this issue. Is there any way to recover from failed auto patch?

[root@ OPatch]# ./opatch auto /s01/app/grid-binaries/PATCH -ocmrf /tmp/file.rsp -oh /s01/11.2.0/grid,/s01/app/oracle/product/11.2.0/db_1
