Multiplying Dependent Demand with MRP run

Has anyone had an issue with dependent demand multiplying with every MRP run (unless you clear tables)? 

I have one item set up as phantom.  The demand flows as it should, creating a work order for the phantom, and a planned order for the dependent item.  However, if I run MRP again, the planned order for the dependent doubles, and triples with a third run, etc.  NO other items duplicate in this way - only the dependent.  Any idea what could be causing this??  **Note: this only happens when using Simple Consolidation method.  I tested using branch relationships and did not have this issue**
