How to handle value added service in Order management other than using CTO?

How to handle value added service in Order management other than using CTO?

Customer Places order with an item to make some modification in comments on that part. But part number does not change.

Parts are already in inventory

Only additional work(modification) to be done, before pick release.

Please suggest
