Problem Summary "Service Activity LOV" does not have a value when trying to create a repair order

"Service Activity LOV"  does not have a value when trying to create a repair order using Depot Repair using "Logistics Tab".   Verified below things  Serviceable item is attached to the billing type.  Billing type has been associated with correct service activity code.  Operating unit profile options are set fine.  Item is Contract coverage and installed base trackable.
