Colorado Occupational Head tax not calculating for Geo code 060350030?

All other state taxes are calculating, including a state miscellaneous, H2 amount I'm not familiar with. One side note, My F07940 file was last updated April 29, 2006 and we're on Vertex 4.0. Does this sound right?

Thanks in advance for your help! Everyone on this site does a great job and probably you don't get enough recognition!
