CS 9.0 Financial Aid: What causes the COD Processing Status to be at 'Staged'?

We recently experienced an issue with our COD processing (DL_OUT) for Direct Loans where the first process errored out, but the loan status on the Origination pages show as 'Transmitted'.  In normal situations, we are able to Reset the Processing Status to P (Generated) on the staging tables (View COD Data), however this particular batch is showing at 'Staged', with no ability to reset the button (greyed out).


Being that the first process is in 'Error' status, we also have not been able to create the XML loan files that need to be sent to the Feds.  I am in the process of using a SQL to update the Processing Status to P but wondered, what is the cause of this issue?  How can it be prevented?  As many years that I've been working in Financial Aid, I have not ever seen this come up before.
