adop phase=prepare fails. I just ran a complete patch session successfully, but now prepare fails.

My last adop session completed successfully, but I did have to set 'flags=autoskip' in order to skip 3 INV forms and 3 PO pll generation errors.  Now I am trying to apply several more patches to resolve the INV Forms and PO issues, but adop is failing the prepare phase saying the one of the patches, 20094809 failed.  Stuck in a vicious circle.  Any ideas on how to get past this and apply my new patches?
