Gross Pay DBA Table

I'm trying to set up a table for a DBA that has tiered rates for the same gross amount. For example- our LTD requires .8 to be multiplied by 67% of the first $2500 and 50% of the next $4000 of the gross pay. Is it possible to set up a table like this for one DBA?
