Move Legacy EBS - Merge Nodes and Relocate DB

Hi EBS Tech Experts,

I have a legacy EBS installation (R11 with RapidClone) that is using a shared APPL_TOP with DB/Concurrent on one node and Web/Forms on the other.  I need to retain the legacy EBS, but the hardware is dying.

Can someone help me identify the proper sequence of steps to:

1.  Move the EBS installation and DB to a single server

2.  Merge the two nodes to a single node on this new server

The server name/IP will be changing during this process.  The OS is under my control so I can keep it on the same OS (Solaris 10) revision.
