Unable to create SR. Unable to sumbit form via CONTACT ME.

I am unable to create an SR.

The PRODUCT TYPE field is blank / empty.

I was able to create SRs earlier today.

Also, I tried to submit a form via your "Contact Me" section but the SUBMIT button never becomes available.

HELP!

I need to submit an urgent SR!!
