Appraisal Total Scoring fast formula not being called in appraisal

I have created a Performance Management Plan.

The appraisal template used has a fast formula for Appraisal Total Score Formula using which the Suggested Overall Rating is calculated.

But in my appraisal the Suggested Overall Rating field is blank.

How do I make sure that the fast formula is being called?

In the fast formula I have just hard coded one of the rating_level_id of the Final-Rating Scale

I have double checked the setup and can't find anything missing.
