Usage Interface into Par Count

We have a 3rd party clinical software that is generating usage transactions and we would like to utilize this for replenishment. These locations are set up as pars, and the count is coming in as a Request quantity.  The usage information is coming back as the smallest unit of measure. For our inventory items this isn't a problem (Becomes an MSR and replenished). They will come in as a par count and be reordered in that standard unit of measure.

My issue comes into play, are for items that are bought in a case or larger UOM than is available and not stocked in the Inventory. We are ordering the case and boxes and are thus being overstocked.
