Debrief Quantity differs from Parts Requirement Quantity

Hello,

We have an issue where, when you have created a parts requirement for the Task for a part with quantity 1, and completed IR/ISO. When you are ready to debrief the task - in the Debrief window, Material Tab -> Click on 'Parts Requirement' button. System is pulling the part but with a quantity of 2. This happens in both Depot Repair and Field Service. We are on 12.2.4

Can you please let me know what could have been gone wrong here.
