Trying to perform a FPGA upgrade an the jtag command fails

We are trying to perform a FPGA upgrade as part of an SBC upgrade from SCX673 to 7.2.  We are following all the steps in the FPGA upgrade document but when enter the jtag command (jtag /code/PRG-0059-00r2p20_0619_program.evf) it starts to run but then stops between.... Completed EVF record 35000 and Completed EVF record 94000.  We eventually have to turn the power of and then back on to get the SBC to respond again and when we run the show qos version command the QoS FPGA Hardware Revision is now 0.00. 

We've tried replacing all related files on the SBC and then rerunning all of the commands a few times but the jtag command always stops and never completes.Can anyone recommend what steps we should take to resolve this FPGA upgrade issue?  Thanks.
