Have a custom XML report that loses its template intermittently in Oracle e-bs. Any advice?

It is apart of a request set.  I have attached logs from two different submissions where one worked and one did not and the program and template setups.  They were 20 mins apart and both submitted as new requests (copy request not used).  Any advice would be appreciated.  I have searched knowledge base articles, etc. but have only found suggestions where the output generation doesn't work at all.   Thank you.
