Oracle diagnostic tool results in failed with several errors?

Hello,

I had run diagnostic for a invoice and get bellow error , it is mentioned to log in iTAR to resolve this issue but i hadnt loged one before and dont know how to log it, can any one of you help me to log in iTAR or help me to resolve this error. It is very urgent,  i have other issues to provide diagnostic for to SRs.
