TypeError: Cannot read property 'length' of undefined

I tried one example for  "SIS Account Entry Applet" to hide field "MainPhoneNumber"  based on "MainFaxNumber" field.I am getting blank screen when applet is loaded.

Please find attached files and let me know what could be wrong.
