ERISA elections (electing to receive electronic plan documents vs paper)

Anyone have a solution or ESU for asking for an employee's ERISA election via JD Edwards Employee Self Service?  We do not have a good method for gathering this information and are looking to add it to our Employee Self Services so we can track it.  I attached the current form we are using.
