adop phase=prepare Fail multiple entries for PATCH context file found

I am now on R12.2.4  follow doc (ID 1617458.1) Section 8: Apply Additional Critical Patches,

when run adop phase=prepare

I got error  :

[UNEXPECTED]Error 1 occurred while Executing txkADOPValidation script on hanyora

[STATEMENT] Please run adopscanlog utility, using the command

"adopscanlog -latest=yes"
