OAK 12.1.2.4.0 GI Patching Failing

I'm on a X4-2 and going from 12.1.2.3.0 to 12.1.2.4.0 and the GI patching is not completing. The logs located in /u01/app/12.1.0.2/grid/cfgtoollogs/opatchauto do not show any issue. Here is the snippet from the GI Update Log. I'm not finding any other logs to assist with why this is failing.
