Missing Communication Icon inToolbar

Has anyone seen this before?  I have a couple of CTI Agents not getting the Communication Icon in the top right but under Tools -> Communications the commands are listed and the agent is able to log/out of the phone.  See screenshot attached. SCOMM Logs sent to Oracle Support via SR but nothing found yet.
