How to apply a surcharge for medical rates

How does one apply an extra value (surcharge) to a rate if variable rates already exist on that standard rate?

This is an additional value to be applied to medical plan options that include a spouse and need the value applied to both the standard rate and the variable rate. Currently the surcharge exists as it's own variable rate and is added to applicable standard rates.
