Subscription Fulfillment Program erroring and not creating sales order from Service Contract

We are using Service Contracts and looking to have a sales order created for a fulfillment schedule by running Subscription Fulfillment Program.  Program completes normal but does not create sales order.  Following message is displayed:

Message      : Error messages from Order Management

Message      : Contracts will not get called as contract template id and source document type id are not provided.

Message      : Error in Line: 1 :Please enter required information - Line Type

Message      : Encountered error condition in Package OKS_OC_INT_KTO_PVT Procedure CREATE_O_FROM_K


Confirmed that OM Profile for Salesperson has a value set (No Sales Credit). 
Why are our orders not getting created?  I am not finding clear hits on the 2nd and 3rd messages above.
