EM 12.1.0.4.0 server details not reported by agent

Of the servers monitored by my EM installation all have configuration details complete, but one.  The server in question has the one agent that not 12.1.0.4.  When I attempt to upgrade the agent I get a message that the prerequisites are not met, but the only detail I could find was output indicating that the /etc/inittab was possibly corrupt.  I cannot view this file on any of the servers, but on one server where the server configuration details are complete, /etc/inittab has the below permissions
