Duplicate Sales Order Number

Hi,

Came across a strange situation today when we found two sales orders with same order number (DOCO) and same order type (DCTO) but the orders were placed at different times one in way back 2014 annd other is newly created a month back.

What could be the possible scenarios / bugs can cause this situation?

We are on 9.0 release.
