Trigger fire three times

I have a dynamic trigger on table PER_PERIODS_OF_SERVICE.The trigger is associated with procedure having parameter and the parameters are inserted in the custom table.

When we are entering the actual termination date and leaving reason in end employment screen and terminate the employee.The table PER_PERIODS_OF_SERVICE updated.As soon as the employee got terminated the custom table populated three times.

Please suggest the reason.
