Unable to open Installed Base Instance Search page

In a recently upgraded instance from 11.5.10.2 to R12.2.4, I have added "Installed Base Agent Responsbility Menu" as a sub menu to a custom menu that is associated with a custom responsibility. Then I could open Installed Base Search page by clicking the function "Item Instances" without any issues. But after couple of re logins it is not working and throwing an error "You do not have access to Instance Search Page, Please contact System Administrator."  I am not getting why it worked initially and not now. Please let me know if any other step is missing.
