Steps to Upgrade RAC Database from 11.2.0.3 to 11.2.0.4 on Solaris

We need to upgrade our Grid infrastructure version 11.2.0.3 to 11.2.0.4 and database version from 11.2.0.3 to 11.2.0.4.5. This is a 2 node Oracle RAC Database on Solaris 11 Sparc, 64-bit, machine.

Kindly share the relevant documentation and patch numbers for upgrade.
