AddNode.sh succeeds, but cluvfy reports PRVF-4038

We had a catastrophic system failure on a server (FMPS2) server.  The SAs did not have a valid backup of the /app directory.  So, I’m attempting to add the lost node back to the cluster now that the system has been restored to normal functionality. First, I made sure that FMPS2 had been completely removed from the cluster.  Then, I sourced the cluster profile (.crs_profile) which sets the ORACLE_HOME, LD_LIBRARY_PATH, and a couple of other parameters.  Then, I run the addNodes.sh in the grid home. Everything goes fine.  FMPS2 is added to the cluster and everything comes up roses.
