Cannot interface additional line from CA to OM

Even after Customer Contract is already interfaced to sales order (Order Management), users frequently need to add line to existing contract in CA module and it needs to interface to existing sales order

It's working fine in one instance as attached "Note CA to OM.docx"

However, it failed in another instance as attached "Issue_in_PROD_20151107.pptx.docx"

 

We have no idea why system behavior differ among instances.

Any condition or criteria for this functionality? please advise.
