Create document Web form having bug

Dear Guros,

We are facing issue on Oracle - Create document web form on New Instance it showed blue color on top although functionality going fine but it look like odd present.

Attach are the screen shot for this issue.

Please note : we are deployed various patching activities as well for testing purpose.

Is this bug so how we remove it ????
