While trying to do root mirror in Solaris 8, its failing.

I have a customer whose both root disk was replaced due to some hardware issue. In new two disks, one disk is used to restore the old OS from the tape and is working fine. The Operation System is Solaris8

While trying to mirror, I managed to create metadb in  working OS disk. While trying to create metadb in second disk it gives error output and metadb cannot be created. After a few minutes server gets hung and got panic rebooted as shown below:
