How do you find out where bad data is in 12cc for Patching DP for Oracle Home info

I was looking at doc 1477422.1 and was trying to figure out how I can fix an issue where when you create a patch plan and you go to the section for "Database Oracle Home Credentials" it shows multiple entries even though you are patching only one home.  There has got to be a way to clear up the corrupted data of the OH in 12cc somehow.  Usually I drop and rediscover all the homes but in this case that doesn't seem to help.

For example, if I patch a RAC 11.2 home the Database Oracle Home Credentials section shows me 5 homes even though it should only show 1.  What's more, if the "Use" option is chosen for "Oracle Home Preferred Credentials" then if shows that only the first home of the 5 will use the credentials and the other 4 show a status of not set as well as no entries for either the "Host" or the "Instances" field.
