Debrief Form Grayed out

Hi Team,

Need your support for the debrief task, somehow not sure what setup I missed out which makes me unable to work on debrief form. Although I have attached the issue which i am facing.

Before that would like add here, already I have defined the IB transaction types, task status also with transitions please advice if there is any debrief statuses or transitions needs to be setup.Same issue facing on the HTML form also.
