Obsolete version of afptch.odf file being used by the 12.2.0 patch

Over the last few months I have successfully completed numerous trial upgrades from 12.1.3 to 12.2.4.

I recently decided to run Patch Wizard in our 12.1.3 environment and apply all the "Recommended and Consolidated Maintenance Patches". Since then I have encountered the following upgrade issue;

1) During the application of the 12.2.0 patch (10124646) I am getting this error message;

FAILED: file afptch.odf on worker 3.

(The worker log file has this message);
