unable to create item post database upgrade from 11.2.0.3 to 11.2.0.4

Hi,

EBS 12.1.3 migrated to Exadata and Exalogic.
The database upgraded from 11.2.0.3 to 11.2.0.4
The "DATABASE PATCH FOR EXADATA (JUL2015 - 11.2.0.4.17) : (21025821)" is applied

Post migration we are unable to create item in form INVIDITM.fmx . Saving of item creation is not happening.

As per Doc ID 1640733.1 the patch 17501296 will fix the issue. This patch bug is included in the "DATABASE PATCH FOR EXADATA (JUL2015 - 11.2.0.4.17) : (21025821)" This is already been applied.

but we still have the issue. The opatch lainventory output from both the database RAC node is uploaded
