FSG issue with Report Manager Template to Excel

I have an issue that has just cropped up with a template for an FSG report to export to Excel 2013 via Report Manager.  I created a template, it has all of the columns and data variables from the FSG report.  I uploaded the template to Oracle.  But when I run the report it only shows the first column of the report without formatting and without the other twelve columns of data.  Any help would be greatly appreciated.  We are EBS 12.1.3
