Corrective Action failed.

I created an OS corrective action and set it to run on the warning threshold. When the warning level is crossed, I see in the event that the update tab shows a message of "Execution created for Corrective Action OPT ORACLE CLEAN UP" . Then a few seconds later I get "The execution for Corrective Action OPT ORACLE CLEAN UP failed.". Where can I find what failed?

I am on OMS 12.1.0.4
