How to delete duplicate position in position hierarchy?

We use position hierarchy for PO approval in 12.1.3.  There is a position that, when queried in the hierarchy, shows up twice.  Once with a holder, and once without.  Is there a way to delete the extra position?  This position is only assigned to one employee, and users are getting confused.
