Custom Export Item/Portfolio scope

I am able to get the custom export work when export an item in Form but unable to do it in scorecard.  Did I do something wrong on the tag?  Below are the order I put the tags and it works..

<PPM:ForEachInputItem>

..........

      <PPM:CategoryValue PPM:name="Test123"/>

...........

</PPM:ForEachInputItem>

...........

But when I try to use it in scorecard, I only get the summary value.  I tried to change the tags but kept getting error when I create the template.

Did I use a wrong tag or missing any required tags or put the tags in wrong order?
