Attention: To align with new corporate security policies, and to match policies adopted by other communities within Oracle, the My Oracle Support Community access requirements will change beginning September 18, 2025. Found few issues with ACA Phase 2 patch

Hi All,

After applying the patch, I found out following issues with the patch.

1: 1095-c oracle form format is different from IRS. Is it acceptable by IRS?

2: City and State were not printed for employer and employee address fields on the form?

3: part 2 Line 15 on 1095-c Form should display lowest cost of the self-only plan, but its showing the lowest cost of the equivalent benefit option

For example: Our lowest cost plan is Blue Advantage HMO, the premiums for Employee only $ 150 and Employee + Spouse $175

Employee enrolled in HMO Illinois Employee + Spouse plan.

Line15 shows $ 175 instead of $150 ( I think its taking corresponding option from the lowest cost plan)
