ACA 1095-C Indicator Codes Lines 14 and 16

In Oracle, what is driving the selection of the indicator codes for lines 14 and 16 on the 1095-C form for the employees?

Our HR dept is expecting to see code 1A on line 14 for all employees who were here for the full year, but they're seeing some employees with codes 1B or 1D. For example, we looked at two employees, both single with no dependents -- one had code 1A and the other had code 1B. What settings caused 1B to be selected? 

For line 16, they're expecting to see code 2C for all employees who have elected coverage, but the line is blank for employees with code 1A on line 14. Code 2C appears to only show up for employees with codes other than 1A on line 14. I did see that the setup instructions for the first patch mentioned that the code for line 16 was determined by the setting for the Safe Harbor method, but the IRS instructions seem to indicate that 2C is used if the employee is enrolled in coverage, regardless of the safe harbor selection.
