Telesales eBusiness Center form issue, FRM-40105: Unable to resolve reference item

Hi All,

We upgrade the Application from 12.0.6 to 12.1.3, i am getting error while opening the Telesales eBusiness Center form. I am getting  below errors:

1. FRM-41045: Cannot find item: invalid ID

2. FRM-40105: Unable to resolve reference item AST_DH_LOCATION.PARTY_SITE_ID

3. FRM-40105: Unable to resolve reference item AST_DH_LOCATION.ADDRESS_FAF

We are not able to resolve the issue. We raised a SR but not get any proper solution from the Oracle.
