Affordable Care Act - Why would the offer of coverage codes and safe harbor codes be blank?

I have the Offer and Coverage setup on the Plan Master for the self insured plans, and on the employee for the non self insured plans and when I run the 1095-C workfile build I get blank offer of coverage codes and blank safe harbor codes.  I figure I must have something not set up correctly, but I can't figure out what.  Could someone give me some suggestions as to where to look please.
