12.2 Rapid-Install fails on Applying one-off patches step with Error "Fatal Error: TXK Install Servi

The rapidwiz installer keeps giving errors on the Applying one-off patches step with "Fatal Error: TXK Install Service"

This is a 2 tier install. I've followed Doc ID 1596433.1 - R12.2 : How to Create the Stage in Preparation for Installation.

I've download and I'm using the latest startCD #50 . I've completed the database tier successfully and the new database(Vision) and listener are up and running.

On the appsTier, all pre-reqs are green and check marked. I keep getting this error on this particular step of when it attempts to Apply the one-off patches:

Has anyone run into this issue before?
